In order that our parish can better serve your needs, we ask that all parishioenrs register with the parish.  This can be accomplished by filling out a parish registration form (see below) and submitting it to the parish office one of several ways:

• Bring it to the parish office.  During business hours, turn it into a member of the parish staff.  Outside of business hours, deliver it via the drop box slot in the parish office front porch.
• Bring the completed form to mass and put it into the offering plate.
• Mail the completed form to the parish office at this address.
• Fax the completed form to the parish fax number.

If there are any questions about completing or submitting the registration form, contact the parish office.
 

Forms can be picked up in the back of the church, from the parish office (during business hours), or by clicking below on the Church you are registering with.   Please fill out all information and submit to the office.
